This form must be submitted once per child to request registration for Affirmation Dance hosted by Martinson Health. 

Once received, our office will contact you within 1-2 business days to complete, arrange payment, and confirm registration. 


Dancers will be assessed and may be placed into a different class, taking into account their ability, in addition to age and experience.
Every child at Affirmation Dance has an opportunity to have a dance number choreographed for them to attend competition in Camrose at the end of the year (at additional cost).

 

All fees are due by September 1st. A valid credit card must be put on file if paying in two instalments.

40% of fees are due upon registration to hold the spot; all remaining fees must be paid by September 1st and outstanding amounts will be charged to the credit card on file.

Fees can be paid by e-transfer to office@martinsonhealth.com, and this is the preferred method to help us avoid surcharges.

There are no refunds for dance fees paid.
